Telehealth Risk Assessment Screening

We have expanded our services for this year to offer telehealth eye health risk assessment visits.

During the telehealth visits, clients will participate in an eye health questionnaire to determine whether they are at risk for eye disorders or diseases.

Eye care resources are available to qualifying clients.

Eye Care Resources may include: Eye Exam Voucher*; Eyeglasses Voucher*; Exam and Eyeglass Voucher for Children*

*All resources are based on program eligibility*
